Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Practice: L. Pernoud LLC, d/b/a/ Maison Fleur Integrative Relational Therapy
Effective Date
Effective date: November 3, 2026. This notice describes how health information about you may be used or disclosed and how you may access it. Please review it carefully.
Our Responsibilities
Maison Fleur is required to protect the privacy and security of protected health information (PHI), furnish this Notice, follow its terms, and provide legally required breach notifications. PHI includes information relating to your health, care, and payment.
Uses and Disclosures Without Written Authorization
We may use or disclose PHI for treatment, payment, and health care operations, including coordination with other treating professionals, billing and claims, scheduling, documentation, quality review, and compliance. Other uses or disclosures may be permitted or required by law, including certain public health, health oversight, judicial or administrative proceedings, law enforcement, mandatory reporting, and serious-threat circumstances. The precise legal requirements and safeguards govern each disclosure; not every disclosure of adult abuse is automatically reportable.
Uses Requiring Authorization
Other uses and disclosures require written authorization unless otherwise permitted or required by law. Authorization is generally required for most uses and disclosures of psychotherapy notes, marketing involving PHI, and sale of PHI, subject to applicable exceptions. You may revoke an authorization in writing prospectively, except where we have already relied on it.
Additional Protection for Certain Substance Use Disorder Records
Some substance use disorder treatment records are protected by federal law at 42 CFR Part 2. If we receive or maintain records subject to Part 2, we will use and disclose them only as Part 2 and HIPAA permit. Your written consent may be required, including for treatment, payment, and health care operations, unless an exception applies.
Part 2 records, and testimony relaying their content, will not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you without your written consent or a court order that meets Part 2 requirements.
Information disclosed under HIPAA may be redisclosed by the recipient and may no longer be protected by HIPAA, unless another law, such as Part 2, limits redisclosure.
Your Rights
Subject to applicable law, you may request access to and copies of records; request amendment; request an accounting of certain disclosures; request restrictions; request confidential communications by alternative means or locations; receive a paper copy of this Notice; and receive required notice of a breach. We generally must agree to a requested restriction on disclosure to a health plan for payment or health care operations when you paid for the service in full out of pocket and disclosure is not required by law. We must accommodate reasonable requests for confidential communication. Some rights have exceptions and procedures.

You may complain without retaliation to the Privacy Contact: Maison Fleur Integrative Relational Therapy, 1714 Deer Tracks Trail, Suite 215, St. Louis, MO 63131; (314) 246-9181; lily@maisonfleurtherapy.com.
