Good Faith Estimate
You have the right to receive a Good Faith Estimate explaining how much your care is expected to cost.
Under federal law, health care providers must give clients who do not have certain health coverage, or who are not using it, an estimate of expected charges before care is provided.
Your Rights
You may ask for a Good Faith Estimate when you schedule a service or at any time before scheduling.
If you schedule at least 3 business days ahead, you should receive a written estimate within 1 business day of scheduling.
If you schedule at least 10 business days ahead, you should receive a written estimate within 3 business days of scheduling.
If you ask before scheduling, you should receive a written estimate within 3 business days of your request.
If a bill is at least $400 more than the estimate from that provider, you may be able to dispute it.
Please keep a copy or picture of your Good Faith Estimate.
Requesting an Estimate
To request an estimate from Maison Fleur, call (314) 246-9181 or email lily@maisonfleurtherapy.com. Because the frequency and length of therapy vary, an estimate is not a guarantee of the total length or cost of treatment. You may ask for an updated estimate at any point during care.
